Write down what you eat and how much you exercise each day. This can help you lose weight and keep it off. It will help you track your progress. And, it can show you where you may slip up. That makes it easier to avoid setbacks.

Keep track of every meal or snack you have, no matter how small. Be sure to write down:

  • What foods you ate and how much

  • Anything else you feel is important (where you were when you ate, your emotions, etc.)

For physical activity, write down:

  • What activity you did

  • The intensity level

  • How long you exercised

There’s also space to write your weekly goals. If you have questions or concerns about your weight-loss plan, share your journal with your doctor.

Download the Food and Activity Diary (PDF format).
