To manage your high blood pressure, you need to stick with your doctor’s treatment plan. This means following his or her advice about lifestyle changes and medicines, if prescribed.

Making an Action Plan can help. Use it to write down:

  • The diet plan you’re following
  • Your physical activity plan
  • The medicines you take

Print the 1on1health High Blood Pressure Action Plan (PDF Format). Take it to your next doctor’s visit. Go over it with your doctor. Have him or her help you fill it out. Review your Action Plan at every visit and keep it up-to-date.
